1. REPEAT VIOLATION: Observed 3 ripped gaskets in the middle deli-line cold holding unit. Replace torn gaskets. 2. Observed ice build up around walk-in freezer unit door, preventing door from closing all the way. Clear away ice and service unit as needed. Ice around door cleared away during inspection, CORRECTED ON SITE. 3. Observed ice build up on the floor of the walk-in freezer unit. Clear away ice to prevent possible injury.

1. Water heater has been replaced, send in Spec sheets for the heater and/or Plan Check - Equipment Application for review if not a one-to-one replacement. 2. Facility is offering catering, turn in caterer's application to add program to the facility.

1. REPEAT VIOLATION: Observed 3 ripped gaskets in the middle deli-line cold holding unit. Repair/replace gaskets. 2. Observed water build up in beer refrigeration unit. Clean/clear and service unit. Temperature was observed to be at 45*F.

1. Observed damage to the back door entry way. Repair doorway. 2. Observed damaged floor tiles in the mop sink. Repair mop sink.

Observed two in-direct connections draining into buckets: - Fan of the walk-in refridgeration unit - Espresso machine at the bar. Have waste connections draining into floor-sinks. Maintain a 1-inch air gap for all connections in case of possible sewage back up.

Observed CO2 tank to not be secure, have chain securing pressurized tanks. CORRECTED ON SITE.

Observed each hand sink to be using a bottle of soap, not wall mounted hand soaps. Discontinue practice and resume use of wall mounted hand soaps.

Observed dried dairy on the spout of the milk frother in the beverage station. Regularly clean affected area to prevent bacteria growth. 3. Observed a tear in the middle deli-line cold-holding unit in front food prep area. Replace/repair gasket.

Observed the following items with out lids on properly in the walk-in refrigerator and freezer: marinara sauce, vanilla ice cream with ice cream scoop resting inside. Properly seal food items to prevent possible contamination. CORRECTED ON SITE.

Observed mesh strainer in hand sink at bar area. Hand washing sinks must not be used as a dump sink.

Observed three sanitizer buckets at 0 ppm quat. Maintain 200-300ppm. CORRECTED ON SITE.

Observed a dead cockroach suspended in a spiderweb under the wire shelving in the food storage area. Old cobwebs are visible, no other signs of pests. Clean affected area to prevent further pests.

Observed a metal bucket over flowing with water, bucket is being used to capture drainage from ice container at bar. Please have proper drain line leading to floor sink for drainage.
